Key Responsibilities

  • Assess, plan, implement, monitor /evaluate client’s medical, social and behavioral needs.
  • Handle an independent caseload.
  • Create and implement treatment plans.
  • Ensure accurate documentation/compliance with MaineCare/best practice standards.
  • Provide education/ support to client across a variety of settings (home, school, work, etc.)
  • Skilled at making referrals, monitoring/follow up activities to ensure coordination of care.

Requirements

  • Bachelor’s in social work, psychology, health and human services, or related field. Current MHRTC |MHRTC Eligible a plus!
  • 2+ years’ experience in case management.
  • Strong interpersonal, clinical, and electronic documentation skills.
  • Ability to advocate/strengthening family/community internal/external resources to successfully meet client needs.
