Key Responsibilities
- Assess, plan, implement, monitor /evaluate client’s medical, social and behavioral needs.
- Handle an independent caseload.
- Create and implement treatment plans.
- Ensure accurate documentation/compliance with MaineCare/best practice standards.
- Provide education/ support to client across a variety of settings (home, school, work, etc.)
- Skilled at making referrals, monitoring/follow up activities to ensure coordination of care.
- Compassionate, enthusiastic, strengths-based approach.
- Skilled communicator/team player, passionate about client care.
- Independent problem solver who seeks help when necessary.
- A strong communicator, self-starter, and team player with a sense of humor and heart
- Strong boundaries/ethical direction.
- Bachelor’s in social work, psychology, health and human services, or related field. Current MHRTC |MHRTC Eligible a plus!
- 2+ years’ experience in case management.
- Strong interpersonal, clinical, and electronic documentation skills.
- Ability to advocate/strengthening family/community internal/external resources to successfully meet client needs.

Ideal Candidate (Who You Are)
- A licensed clinician in Maine (LCSW, LMSW-CC, LCPC, LCPC-C, or LMFT)
- Experienced – at least 3 years in mental health settings
- Skilled in working with diverse, vulnerable, and marginalized populations, both with adults, young adults, and youth
- Comfortable with EMRs and familiar with Microsoft 365
- A strong communicator, self-starter, and team player with a sense of humor and heart
- Provide individual outpatient therapy to adults and youth
- Conduct clinical assessments and develop individualized treatment plans
- Utilize trauma-informed, evidence-based approaches (e.g., CBT, DBT, MI, CPT, etc.)
- Address issues such as serious mental illness, aging-related concerns, trauma, and identity
- Participate in regular supervision and ongoing professional development
- Maintain accurate medical documentation in compliance with regulatory standards
- Collaborate with other healthcare professionals to ensure holistic care for clients
- Monday to Friday (flexible; set your own schedule)
- Full time fee for service model
- Fully remote or hybrid, as preferred
- Must reside in Maine
