Community Choice Behavioral Health provides Adult Case Management and licensed Outpatient Therapy

Adult Case Management

CCBH provides Adult Case Management services for individuals to provide paths toward independence and access to resources to achieve stability and achieve their unique life aspirations as they engage with mental health services and other supports within our communities. Our case managers are a trusted resource for our clients as they continually assess needs, coordinate referrals, and provide clients with paths and resources that will lead to greater independence.

We can help adults thrive at home, at work and in their communities by:

  • Connecting with natural supports and community resources.
  • Helping navigate the systems of behavioral and medical care.
  • Formal and informal vocational support
  • Coordinating services and streamlining communication between multiple providers.
  • Access community resources and benefit programs
  • Develop and monitor individualized client driven service plans

MaineCare eligibility guidelines for community integration services are required, please contact 207-640-4400 for more information.

If are not currently enrolled in MaineCare a limited amount of grant funds are available for adults who qualify under community integration case management eligibility requirements.

Outpatient Therapy

CCBH provides licensed Outpatient therapy for individuals to support healing, improve functioning, and lead fulfilling lives. Our licensed therapists use evidence- based practice and trauma- informed care in order to provide clinical assessment, counseling, and necessary therapeutic services to adults living in Maine. Outpatient therapy services help clients deal with stress, trauma, impulse control, personality disorders, eating disorders, behavioral, social, and family problems. We offer individual treatment to address these issues and others which may be creating barriers to clients living a life they envision for themselves.

Diagnostic assessment appointments are also available in order to determine eligibility for section 17 Community Integration Services.

Children & Adolescent Services

Community Choice Behavioral Health provides Children & Adolescent Outpatient Therapy

Children & Adolescent Outpatient Therapy

CCBH Outpatient team works with youth 11 to the age of 18 who present with behavior issues, anxiety, depression, or trauma. Services are prioritized based on the child’s level of need. Services include the initial assessment, treatment planning and on-going therapy

Our team utilizes multiple Evidenced-Based treatments that are based in research to provide effective results. The team is also trained in trauma informed care and respond with methods that take trauma and trauma response into consideration.

The Child and Adolescent Outpatient Clinic is ideal for children and teens who:

  • Are struggling with mental health conditions including depression, anxiety, OCD, phobias, bipolar disorder, and trauma.
  • Need skills to better understand and manage their symptoms
  • Seek testing to evaluate difficulties in educational, home, or social settings
