Key Responsibilities
- Assess, plan, implement, monitor and evaluate client’s medical, social and behavioral needs
- Handle an independent caseload
- Create and implement treatment plans
- Ensure accurate documentation/compliance with MaineCare/best practice standards.
- Provide education and support to client across a variety of settings (home, school, work, etc.)
- Skilled at making referrals, monitoring and follow up activities to ensure coordination of care

Requirements
- Bachelor’s in social work, psychology, health and human services, or related field.
- 2+ years experience in case management
- Strong interpersonal, clinical, and electronic documentation skills.
- Ability to advocate for strengthening family internal and external resources to successfully meet child needs.
