Key Responsibilities

  • Assess, plan, implement, monitor and evaluate client’s medical, social and behavioral needs
  • Handle an independent caseload
  • Create and implement treatment plans
  • Ensure accurate documentation/compliance with MaineCare/best practice standards.
  • Provide education and support to client across a variety of settings (home, school, work, etc.)
  • Skilled at making referrals, monitoring and follow up activities to ensure coordination of care

Requirements

  • Bachelor’s in social work, psychology, health and human services, or related field.
  • 2+ years experience in case management
  • Strong interpersonal, clinical, and electronic documentation skills.
  • Ability to advocate for strengthening family internal and external resources to successfully meet child needs.
