What do Mental Health case managers do? Mental health case managers help individuals develop coping skills and maintain a stable life. They assess individuals to evaluate the state of their mental health, develop a plan of care to assist individuals with their goals. Develop goals such as housing, employment, connection to other service providers and community resources. They assist with the organization of medical records and appointments and connecting individuals to transportation through ride share programs.![]()
Our case management programs offer Targeted case management to adults and transition-aged (13-18) adolescents under Section 13 rules & Regulation established by the State of Maine, Adult case management through Community Integration Services (CIS) through section 17 with MaineCare insurance. ![]()

ABBY LAPLANTE
MPH, MHRT/C, Case Manager
Abby LaPlante (she/ her) is a clinical case manager at CCBH who enjoys building authentic connections with her clients and helping them work toward their goals. She believes in seeing each person as a whole individual and working collaboratively to find solutions that fit their unique needs. Abby has worked in case management on and off since 2018 and brings a compassionate, individualized approach to every person she serves. She has experience supporting individuals experiencing homelessness and substance use disorder and is passionate about helping clients take meaningful steps toward stability and growth.
Abby holds a bachelor’s degree in Anthropology, a master’s degree in Public Health, and is certified as a Mental Health Rehabilitation Technician – Community (MHRT/C) by the State of Maine. When she’s not working, you can find Abby hiking with her dogs, camping, or enjoying just about any outdoor activity.![]()
Abby has been a valued employee of CCBH since 2024, supporting adults in the southern Maine area in both York and Cumberland Counties.
