  Key Responsibilities
  • Assess, plan, implement, monitor and evaluate client’s medical, social and behavioral needs
  • Handle an independent caseload
  • Create and implement treatment plans
  • Ensure accurate documentation/compliance with MaineCare/best practice standards.
  • Provide education and support to client across a variety of settings (home, school, work, etc.)
  • Skilled at making referrals, monitoring and follow up activities to ensure coordination of care

Community Choice Behavioral Health provides Community Integration Case Management services and Outpatient Therapy with an emphasis on Trauma-Informed care practices. We are currently looking for a Per Diem Case Manager to add to our growing community integration services team, adult section 13, and TCM services for youth. Services offered primarily through telehealth, with some in-person meetings within the community as needed, according to client preference. This is a fee for service position with flexible hours. Experience working with populations with co-occurring disorders/serious mental illness/behavioral challenges, homelessness, or transitioning youth is strongly preferred. We pride ourselves on providing compassionate, comprehensive, and individualized treatment services and are excited to bring this much needed service to Maine communities.
